Formsite
· #392 most-usedBuild forms, collect responses, and act on every submission automatically
Formsite is a professional online form builder that lets teams create surveys, registration forms, order forms, and complex multi-page applications with conditional logic — no coding required. Connect it to Actionist and your agents can retrieve new form results within about a minute, pull submission data for any form, update result statuses, manage forms programmatically, and feed responses directly into the CRMs, spreadsheets, and workflows where your team already works.
Eliminates manual work. Agents eliminate the manual cycle of checking Formsite for new submissions, copying response data into CRMs and spreadsheets, and following up on registrations or order confirmations by hand.
What your Formsite agent runs on autopilot
A week of scheduled jobs your Actionist agent will execute on your behalf.

Lead captured in Formsite, routed to CRM in under a minute
When a prospect submits the contact form, the agent extracts name, company, email, and message, creates a HubSpot contact and deal, scores the lead by company size field value, and posts a notification to the #sales-leads Slack channel. The rep sees the lead with full context before Formsite sends its own notification.

How to plug Formsite into Actionist
Pick the connection method that suits your environment.
Connect Formsite by generating an API token from your account settings. The token gives Actionist read and write access to your forms and results.
Log in to Formsite, open any form, and go to Settings → Integrations → Formsite API. Your Access Token, server, and account directory are all listed here.
Note the Access Token, the server prefix (e.g. fs30), and your account directory. Treat the token like a password — anyone with it can read and write all your form results.
Enter the token, server, and account directory in the Formsite connection fields and click Test connection. Actionist makes a read-only call to confirm access before any actions run.
